What Is An Affidavit?

An affidavit is a voluntary, sworn written statement of facts made under oath or affirmation before a commissioned notary public (or other public official), and by signing it, the affiant (the person making the statement) verifies that the information is true and accurate.

Once this is complete, the notary will witness your signature, complete a “jurat” (a certification stating when and where you swore to the document), and affix their official seal. Without this step, the affidavit is legally invalid and may be dismissed in court or by government agencies.

What to Bring

Bring your unsigned document and a valid, government-issued photo ID. Do not sign the document beforehand. The signature must happen in notary’s presence.
